Seniors' Community Connector - Social Prescribing Program - Williams Lake

The Social Prescribing Program supports seniors to access non-clinical, community-based services to prevent or delay frailty by fostering resilience and social support using a comprehensive, strength-based approach.
The Community Connector is a non-clinical role with the goal of identifying seniors who may be isolated, are at risk of frailty and needing help accessing non-clinical, social supports and services within the community. Through “Social Prescribing,” issues such as social isolation, housing concerns, food insecurity or just the desire to be more engaged with community, can be addressed. Depending on what the participant needs (just information vs a comprehensive wellness plan involving multiple areas of support) services are offered.

Accessing the program has no cost, and referrals can come from health care professionals, community based senior services, family or individuals can self-refer. To access the program complete the referral or call or email.
