Interdisciplinary Outpatient Rehabilitation Program (IORP) - North Island Hospital, Comox Valley

- Preventing hospital admissions
- Achieving client-centered functional goals
- Supporting earlier discharge from acute and rehabilitation care
- Providing access to specialized interdisciplinary services closer to home and transition planning
- Recent complex orthopaedic surgery or trauma
- New or longstanding amputations requiring functional goal support or prosthetic training
- Movement disorders with new functional impairments (e.g. Parkinson disease, Functional Neurological Disorder)
- Neurological injuries or diagnoses causing cognitive, language, or physical impairments (e.g. stroke, acquired brain injury, spinal cord injury, Guillain-Barré syndrome)
Program Acceptance Criteria:
- A referral IS required (providers see below); please speak with provider about the exclusion criteria
- Individual must be 18 years of age or older, and reside in a Northern Island Health community
- Client understands the expectations of the program and agrees to being referred to the program
- Client requires at least two of: Occupational Therapy (OT), Physiotherapy (PT) or Speech-Language Pathology (SLP)
- Client has identified functional goals that are appropriate for this time-limited, specialized interdisciplinary outpatient rehabilitation program
- Client demonstrates “rehab readiness” including:
- They are medically stable and safe to participate in light to moderate intensity exercise, if applicable
- They are able to attend multiple days per week for short-term period and able to tolerate 2 hours of therapy, in addition to travel time
- They are able and willing to actively participate in a rehabilitation program, and will demonstrate carryover of new skills between sessions
